At Park Plaza Hospital Diagnostic Imaging Center, we strive to ensure that every patient receives an appropriate level of service. Our caring staff will work to make your experience as comfortable and easy as possible.
On the day of your appointment, you will need the following:
· Photo identification
· Insurance card
· Prescription from your doctor’s office with diagnosis*
· Form of payment for co-payment or co-insurance, if applicable
*Some insurance companies also require pre-authorization or a referral for your visit. Please check with your insurance provider to see if this is necessary.
Prior to your appointment, we will need to collect some paperwork from you. We will provide these forms for you to review and complete on the day of your appointment. If you prefer to review and complete these forms at your convenience, you can print them off at home and bring them with you to your appointment.
